Participate with other staff in carrying out established therapeutic and rehabilitative program. Attends and participates in staffing sessions of assigned residents and provides information and ideas for the optimal therapeutic intervention of those residents. Assists in the admission, discharge and transfer process.

Dispenses medications as ordered for residents and documents each administration on the MAR, as well as follows up when PRN medications are given. Contacts the appropriate physician to obtain orders for both routine and emergency situations and follows through including accurate documentation of orders received.

Monitors and assesses behaviors, symptoms and progress toward treatment goals for each resident daily and documents in the medical record as appropriate. Maintains Medical Records accurately and completely under the supervision of the Nursing Coordinator. Serves as a member of a treatment team and completes nursing responsibilities for each of their assigned residents (monthly summaries; MAR’s; assisting with proper placement, etc.) Assists in monitoring a clean, safe and therapeutic environment; which includes residents’ rooms, facility vans and proper execution of facility safety procedures. Reports unsafe conditions to appropriate supervisory personnel. Attends, assists with and presents staff in-service trainings. Accompanies and supervises residents to on and off grounds activities. Reports difficult or problematic situations to the supervisor available and documents in the medical record as appropriate. Assists with the provision of orientation and training for new employees. Interacts with residents in a variety of situations in order to encourage involvement in activities related to their treatment and care. Educates residents regarding medications, side effects, illnesses, etc. on a as need basis and documents education provided. Assists LPN staff in assessment of resident’s symptoms- as needed. Performs other duties as assigned (such as- but not limited to): Chart Reviews & Chart Maintenance CLIA Monitoring AIMS Assessment One Touch Orientation & Annual Competency Facilitates monthly group related to medical/psychiatric issues. Shift, Third, 11:00 PM - 7:30 AM

Minimum Requirements


Must have an Associate’s Degree in Practical Nursing and have a current license and 1 year of experience. The employee shall become familiar with HIPAA regulations and do their utmost to protect all protected health information (PHI) and other personal or sensitive information within their trust in the course of state business by applying appropriate safeguards and sharing only the information required to deliver agency services. Any personal or sensitive information overheard or seen by the employee will be kept confidential by not sharing it with others, on or off the work-site grounds. Fully participate in all activities regarding the elimination of the use of seclusion and restraints in patient care including educational requirements, competency development activities, and meeting established performance goals and measures. Must have computer competency in approved systems. Must have excellent customer service skills.
