Resident Care Manager

Location: Vernon Hills, IL (97 W. Phillip Road)

 

PRIMARY RESPONSIBILITIES:

Responsible for the oversightmanagement of the Resident Care Department, including staff management, assessment and nursing care for all residents, remaining within budgetary guidelines, and maintaining compliance with all regulating authorities.  Responsible for overseeinghiring, training, and developing resident care department staff.

ESSENTIAL JOB FUNCTIONS:

  1. Know each resident and family member by taking the time to get to know them.
  2. Engage residents and connect with them in a genuine, compassionate, and memorable way.
  3. Manage the oversight of all resident care.
  4. Assist Life Enrichment Manager with scheduling of all healthcare personnel; assure staffing levels meet established residentpatient ratios, schedule remain within budgeted hours and manage and approve all overtime hours worked.
  5. Ensure the assessment process as required by Pathway Senior Living is completed on every resident.
  6. Ensure the Resident Service Plan is updated quarterly as needs or required services change.
  7. Ensure the quarterly reviews required by Pathway Senior Living are completed in a timely manner and are accurate and appropriate to the resident.
  8. Manage the process utilized by Pathway Senior Living to ensure residents’ medication is managed in the method assessed to be the best process for that resident. Ensure all residents have a self medication assessment completed upon admission.
  9. Ensure all self medicating residents are reassessed if condition change is noted and on an annual basis.
  10. Ensure proper notification of physicians and families for all status changes.
  11. Ensure proper management of monthly weights and vital signs.
  12. Monitor that all residents and staff receive proper TB testing upon admission/hire and annually.
  13. Assist the Life Enrichment Manager to direct, supervise and train staff nurses.
  14. Assist with the completion resident care department employee evaluations in a timely manner.
  15. Assist the Life Enrichment Manager to maintain the nursing budget.
  16. Assist the Life Enrichment Manager to coordinate services with other healthcare professionals.
  17. Assist the Life Enrichment Manager with the required Quality Assurance for resident care.
  18. OverseeManage nursing staff by walking around, checking medication drawers, auditing charts randomly for compliance, checking apartments for cleanliness, talking to staff and building relationships.
  19. Assist the Life Enrichment Manager with the continuing education for staff, CPR certification, body mechanics, privacy issues, and resident rights, etc.
  20. Develop collaborative relationships with appropriate physicians, pharmacy, hospice, home care agencies, local hospitals and community based groups.
  21. Assist with the orientation process for all clinical staff and provide performance feedback to the Life Enrichment Manager as appropriate.

ADDITIONAL DUTIES:

  1. Work with all other departments to ensure proper services are provided to the residents.
  2. Attend daily stand-up meetings and provide resident status updates.
  3. Assist with building relationships in the community to enhance transitional care and build census growth.
  4. Responsible for maintaining current knowledge regarding Department of Public Aid regulations and implementing clinical activities in accordance with these regulations.
  5. Complete all required reports and ensure proper delivery to the appropriate person.
  6. Performs other related duties as assigned or required.

 

SKILLS, KNOWLEDGE, ABILITIES REQUIRED:

  1. Current RN or LPN license valid in the State of Illinois.
  2. Two or more years of staff supervisionmanagement preferred.
  3. Previous experience in assisted living strongly preferred.
  4. Strong clinical, assessment and documentation skills required.
  5. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines.
  6. Excellent interpersonal, organizational and communication skills.
  7. Must have the ability to read, write and follow oral and written direction in the English language at a level adequate to perform the job duties.
  8. Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
  9. Must maintain and encourage a positive work atmosphere by behaving and communicating in a manner that promotes cooperation with co-workers, managers and others with whom contact is made.
  10. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  11. Ability to timely and effectively follow up on all necessary and appropriate resident documentation.
  12. Ability to handle several tasks and respond effectively to unexpected changes, requests and stressful situations.
  13. Must possess the ability to make independent decisions when circumstances warrant such action.
  14. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, polices and procedures, etc., that are necessary for providing quality care.
  15. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  16. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

While performing the duties of this job, the employee is regularly required to stand, bend, talk and hear.  The employee is required to walk and balance.  The employee will occasionally be required to climb stairs.  The employee is required to use hand and finger to touch, handle and feel, and is required to reach with hands and arms.  The employee must be able to lift and/or move objects up to 50 pounds.   Must be in good general health and demonstrate emotional stability.  Specific vision abilities required by this job include close vision, peripheral vision, depth perception and ability to adjust focus.
