Licensed Social Worker

Location: Lansing, KS (Twin Oaks Health & Rehab, 757 W Eisenhower Road)

 

Job Description: Performs duties and implements programs which meet physical, mental, and psychosocial needs of the residents. Is involved in marketing and completes numerous forms on admissions.

Accountability: Licensed Social Worker reports to the Administrator.

Equipment Used: Employee may be required to operate an automobile and various office equipment.

Environment: Exposure to both indoor and outdoor environments.

Duties and Responsibilities: The employee must be able to perform the essential functions of the job with or without reasonable accommodations. Except as specifically noted, the following functions are considered essential to this position.

  • Coordinates all admissions to the facility. This includes readmissions, room moves and patient inquires.
  • Coordinates all discharges. Discharge plans should be written and include involvement with family and community services, as appropriate.
  • May be required to coordinate resident Medicare and Medicaid benefits including beneficiary consent, denials, explanations, utilization, & MS 2001. Also responsible for coordinating VA and insurance benefits for residents.
  • May be required to coordinated resident and family  member grievances and acts as the resident advocate when necessary.
  • Assists residents with admissions and assures compliance with all State and Federal requirements regarding admission and Social Services.
  • Actively participates in care plan meetings and may be responsible for involving residents and family members. Required to participate in Quality Assurance, and other meetings/committees as assigned.
  • Responsible for Social Service progress notes, assessments, and care plans.
  • May be required to coordinate family or resident meetings, and give staff in services.
  • May be required to develop and conduct psycho-social resident groups.
  • Assures personal and staff compliance to all State and Federal regulations including blood borne pathogens, infection control, use of hazardous materials, and fire safety.
  • Assures personal and staff compliance to all Residents’ Rights. 

Licensure Requirements: Minimum of Bachelor's degree in Social Work.  Current State Social Work license. 

Other Training, Skills, and Experience Requirements: Employee must be able to perform in such a way to build good working habits and relationships among all staff. Employee must demonstrated on an ongoing basis the ability to develop and maintain good working relationships with co-workers and other department heads.

Other Considerations and Requirements: This position required an individual with high quality communication skills. Experience with death and dying, depression, family conflicts, and loss of independence is helpful.
