The Patient Care Manager (PCM) will be responsible for managing the services of a multidisciplinary team according to patient needs and the physician orders. The PCM will be available to the patients as well as to the field staff to answer questions and assist with problem solving so that the patients needs will be met in the most appropriate and timely manner. The PCM improves quality of patient care by coaching and intervening to facilitate patient progression through the plan of care developed by the Outcomes Manager. Works with a high degree of independence to ensure optimal, clinical, financial, and satisfaction outcomes are achieved in a timely and efficient manner. Serves as a catalyst to challenge current practices and streamline delivery of services.

Duties:

  • Responsible for selection and retention of qualified staff

  • Set department performance goals, monitor results, and create opportunities for continuous improvement

  • Create an "expectation of excellence"

  • Improve staff performance, development and moral through fair and accurate performance evaluations, daily coaching and mentoring, and/or appropriate corrective action as needed

  • Reward top performers and improve or replace lower performers

  • Manages department merit increases within budget

  • Assists with preparation of Medicare, Medicaid, JCAHO, and QA audits

  • Identify variances in standardized care and care processes to identify areas for improved performance related to patient care and the organization

  • Acts as a positive role model or preceptor for team members that are new to home health

  • Serves as a resource in assisting the field staff with problem solving in order to provide the most appropriate care management for the patient

  • Serve as a clinical leader

  • Acts as a resource and has a working knowledge of Medicare, Medicaid and private insurance guidelines related to admission/service criteria and its proper clinical documentation

  • Assists with the patient visits in emergencies or acute staffing shortages

Minimum Requirements

 

The Patient Care Manager is a RN, PT, OT or SLP who possesses a 2 year degree in nursing, physical therapy, speech/language pathology or occupational therapy from an accredited or approved school. Qualified candidates must also have a current license from the state in which he/she would be practicing. 2 years of previous field clinician experience with home health or supervisory experience in health care required. Must have adult assessment skills as well as excellent verbal and written communication skills, organizational skills, interpersonal and problem solving skills.  Must have a current CPR certification, TB skin test read within three months prior to employment and proof of Hep B vaccination.
