Position Overview:
Responsible for tasks related to managing client cases for individuals seeking HIV prevention navigation services. The Care Navigator engage primarily Men who have sex with Men (MSM) of color with the following activities: 4th Generation HIV Testing and Counseling, Linkage to Care and STI Treatment, PrEP/PEP Education, and Linkage to PrEP/PEP. Additional responsibilities include the provision of Benefits Navigation, Linkage to Services, and outreach through New Media platforms, such as Grindr or Jack'd. The Care Navigator will also be instrumental in engaging MSM during evening drop in programming and at weekend events.

• Participate in targeted outreach of the key population (MSM of color and transgender women) for rapid HIV testing and engagement in prevention services 

• Ensure at risk and newly diagnosed clients are linked to Primary Care for confirmatory testing, STI screenings and PrEP/PEP when applicable 

• Work together with peers, Outreach Staff and Care Manager to improve access to prevention services and retention 

• Conduct follow up outreach with linked clients via various methods (in person, phone, social media, new media, etc) to increase consistent engagement in services, such as primary care, mental health, substance use, etc. 

• Conduct Risk Screening, Brief Intervention, Intake Assessment and assist with case management activities and integrated case conferences 

• Participate in community events, such as health fairs, LGBT events, testing events, etc. in an effort to engage new at risk clients for prevention services 

• Document contract targets in eSHARE, e-ICare and maintain weekly tracking form to document effort 

• Participate in the implementation of the new media plan, including upkeep of targeted dating site professional profiles and social media posting across various platforms as part of the strategic social media plan 

• Participate in weekly supervision with Program Coordinator 

• Participate in reoccurring interagency meetings, as well as external community meetings to improve integrated care and maintain relationships with community partners 

• Perform other duties as assigned

Minimum Requirements


Educational Requirements
• Demonstrated experience providing HIV health education and risk reduction to MSM/Transgender communities of color 

• Demonstrated understanding of PrEP/PEP and HIV treatment in communities with barriers to accessing care 

• Strong socio-cultural identification with the priority populations of this service category 

• Strong organizational skills and verbal/written communications skills 

• Knowledge of issues pertaining to homelessness, substance use, harm reduction, mental illness, transgender healthcare, and LGBT youth 

• Good computer skills (Word, Excel, Outlook, etc) 

• Bachelor’s Degree or Associates preferred, High School Education required 

• Ability to work regular evenings and several weekends a month 

• Flexibility with schedule and travel required due to attending outreach events around the city
