The Clinical Referral Support LPN role is responsible for the coordination and processing of new home health referrals. This includes coordination of infusion, HME, enteral and wound care therapies, that are supported by home health field clinicians. This position operates under the supervision of the Clinical Referral Support Manager, this role is responsible for following the Home Health Referral Intake Process. The LPN reviews referral documentation to establish payer compliance in accordance with Medicare/Medicaid guidelines and commercial payers and identifies the patient’s appropriateness for home health services. This role is fast paced, dynamic and highly collaborative with our patients, referral sources, Sales team members and Home Health branches to ensure timely patient care.

This role supports the organization as a central team and provides coverage from 8am-7pm 7-days a week.

Minimum Requirements

 
  • Licensed Practical Nurse degree from an accredited or approved school is required.
  • At least two years of LPN experience in the hospital or public health setting, working within a clinical intake department, is required.
  • LPN experience in home health is preferred. 
  • Must have a valid NC LPN license.
