The Intake/Benefits Coordinator serves as the initial contact for all persons requesting IDD services from Lakes Regional Community Center.  Essential functions of the position include provision of information and referral, preliminary screening activities, assessment, coordination of eligibility verification of persons referred, follow-up and crisis services to persons pending eligibility determination or admission; coordination of admission activities, documentation, and community education within designated counties. The Intake Coordinator is also responsible for interagency coordination of Medicaid and non-Medicaid services through participation in regularly scheduled meetings (Community Resources Coordination Groups, Advisory Committee meetings, etc.) .  Additional responsibilities are to assist Continuity of Services Department with interest list maintenance and alternative placement options for individuals residing in the community.  This position also assists in determining Medicaid eligibility as well as assists individuals with applying for Medicaid benefits. Duties are carried out in the natural environment of the persons served, when possible.  Must possess driver’s license in order to provide transportation to consumers as necessary.  This position reports directly to the IDD Service Coordination Manager. Position requires flexible hours and schedule, periodic travel within the state of Texas and within the 12 designated counties of the LA, and throughout the LA’s 12 counties when indicated, for the purpose of community education activities.

Salary: $17.70/hourly

Minimum Requirements

 

Qualifications: High school diploma; 2 years working in a social services setting or human services related field, or personal experience with the disabilities population. 
Preferred qualifications: 2 - 5 years working professionally in an office-based setting; experience in human services or social services settings; proficiency in Microsoft office programs and excellent speaking and writing skills.
