General Description of Duties:

The Licensed Practical Nurse (LPN) provides practical nursing care to patients taught in practical nursing programs according to the Colorado Nurse Practice Act.  The LPN approaches his/her tasks in a team based care fashion that supports patients and their families in a self-management, self-efficacy and behavior change.  Adheres to the spirit of the SALUD mission statement while performing assigned duties.

Supervision Received:

Works under the supervisions of the Center Management Director at the home clinic with clinical oversight from a medical provider.

Specific Duties:


  • Provides basic nursing care under the clinical oversight of a medical provider or registered nurse to include:
    • Rooms patients and obtains/ records data pertinent to patient visits such as: vital signs, reason for visit, medical history and social history, current medications
    • Execution of a provider or RN developed patient care plan
    • Inserting and discontinuing intravenous catheters after completing a Board of Nursing approved IV Therapy Course
    • Changing wound dressings
    • Basic cardiac life support
    • Medication administration
    • Assistance with numerous medical, surgical, and gynecological procedures
    • Autoclave sterilization of instruments and adheres to infection control policies and procedures
    • Collects, processes, records, and educates patients on a wide number of medical laboratory tests
    • Performs Provider ordered clinical tests, such as non-stress tests, EKGs, Spirometry, vision acuity, ear irrigation, and other tests/procedures as required
    • Inserting and discontinuing urinary catheters
    • Triage according to standard operating procedures and scope of practice
  • Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers and all other staff to ensure that the patients obtain the highest standards of integrated healthcare 
  • Identifies normal from abnormal in each body system to identify changes in the patient’s condition which is reported to an RN or medical provider for “full” assessment
  • Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to SALUD
  • Follows procedures for the electronic health records system: documents all care given and maintains patient health history appropriately
  • May assist with interpretation, as needed for providers
  • Adheres to all standards, protocols, and laws surrounding patient confidentiality and FQHC regulatory requirements
  • Knows and follows the Nurse Practice Act for Licensed Practical Nurses
  • Maintain patient flow throughout clinic by utilizing a proactive approach and anticipating the necessary supplies and setup for each patient visit.
  • Stores, inventories, and administers immunizations in compliance with all state and federal guidelines
  • May be required to travel to other clinics occasionally based on staffing shortages.
  • Performs other duties as assigned


Minimum Requirements


Qualifications for Appointments:

Knowledge, Skills and Abilities


  • Knowledge of the principles of practical nursing theory and the necessary skills to apply them according to the Colorado Nurse Practice Act.
  • Ability to establish and maintain effective communications with clients.
  • Maintains a high level of emotional intelligence
  • Ability to work in fast paced environment
  • Ability to think critically
  • Excellent verbal and written communication skills
  • Knowledge and understanding of appropriate clinical documentation requirements
  • Ability to speak Spanish preferred.
  • Sensitivity to low income and ethnic minority community a must.


Graduate of an approved School of Practical Nursing.


           Two years LPN experience

  • Primary Care, ambulatory or out-patient care experience preferred


           Licensed Practical Nurse from the State of Colorado, in good standing.

  • Must maintain a certification in Basic Life Support CPR

Bloodborne Pathogen Risk Level:

           High risk for bloodborne pathogens.

Physical Requirements:

Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for prolonged periods of time. Occasionally lifts and carries items. Requires normal visual acuity and hearing. Requires exposure to communicable diseases and bodily fluids. Requires working under stress in emergency situations or irregular hours.
