Social Worker

Job Description


As a Social Worker, you are responsible for implementing services to meet the medical related psychosocial needs of the residents. 

  • Meeting with the new residents and their families, conducting admissions and intake
  • prepare for discharge
  • coordinate with home health as needed
  • order durable medical equipment
  • participate in weekly care plan meetings
  • strong conflict resolution skills
  • Highly preferred a Social Worker with MDS experience and Long Term Care as well as discharge planning skills




Must meet the following minimum qualifications:

  • A Bachelor’s degree in social work or human services appropriate to resident needs
  • 1 year, within the last 5 years, of supervised direct social work experience.
  • Must be trained in recognizing and assessing the emotional and social needs of residents.
  • Must be knowledgeable of community agencies and resources available to meet assessed resident needs. 
  • Highly organized
  • Ability to multi task
  • Effective communication skills
  • Strong understanding of Medicare/ Insurance
  • Must be able to facilitate meetings with families and staff.
  • Good Computer skills.
  • Experience writing care plans and documenting progress, concerns, etc a plus.
