Job Description


PRN at Woodbine Rehab.


This position is responsible for conducting nutritional assessments in a sub-acute/long term care setting to determine dietary needs of individual residents.

Regularly assess and reevaluate residents’ nutritional status inclusive of lab value interpretation, weight changes, dietary intake, and skin integrity.

Collaborate with the interdisciplinary care team in plan of care and family meetings to ensure that residents’ nutritional needs are met.  

Conduct routine assessments as required, visit residents on a regular basis to discuss nutritional problems and recommend appropriate solutions.

Order therapeutic diets and supplementation as appropriate to residents’ nutritional status and clinical condition.

Document findings and recommendations in electronic medical records system and enter pertinent data for MDS processing.





Bachelor's Degree in preparation for registration as a professional Dietician.

Must be a Registered Dietitian with the Commission on Dietetic Registration of the American Dietetic Association.

Previous experience providing nutrition therapy in a clinical setting in acute care (as obtained in dietetic internship).

1-2 years of previous work or dietetic internship experience with geriatric and rehab population in the long term care setting preferred.
