Key Summary and Responsibilities

· Conduct nutrition assessments of newly admitted residents in the Rehabilitation, Skilled Nursing, and Long Term Care setting

·Regularly follow up on records of at-risk residents such as those with wounds or unplanned weight changes

·Document findings and recommendations regarding resident’s nutritional status and enter pertinent data for MDS processing

· Participate in the resident care plan meetings by collaborating with the interdisciplinary team, including nursing, physical/speech therapy, and social work

·Visit residents on a regular basis to discuss nutritional problems and recommend appropriate solutions based on their needs and level of care

·Assess resident weight changes and compile a monthly report

·Serve as a clinical resource to nursing personnel regarding the nutritional condition of residents and their appropriate dietary interventions




  • Bachelor’s degree from an accredited university with major studies in food and nutrition.
  • Completed Dietetic Internship from an accredited Internship program.
  • Registered Dietitian (or RD eligible) with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics
  • 1-3 years of experience with a geriatric and rehab population in the long term care or hospital setting
