Physical Therapist (PT)

Job Description

 

As a Physical Therapist you will be responsible for the following:

  • Identification of needs and delivery of services to residents who require physical therapy intervention in accordance with all Federal, State and facility requirements. 
  • Collaborates with facility staff to promote the highest practicable physical, mental, and psychosocial well being for the residents.
  • Screens, evaluates, and develops physical therapy programs for implementation for all residents, as needed.
  • Provides clinical supervision to Physical Therapy students, LPTAs, and Rehab Technicians in accordance with State guidelines.
  • Develops effective physical therapy treatment plans and goals to meet each residents needs, and produce meaningful functional outcomes.
  • Completes documentation, functional outcome, and billing in an accurate, timely, and legible manner, consistent with published regulations and standards for insurance compliance and reimbursement.
  • Ensures MD orders are obtained according to facility and/or company policy
  • Demonstrates effective oral and written communication skills
  • Demonstrates effective problem solving skills
  • Complies with APTA code of ethics, State Practice Act for Physical Therapy, and HIPAA guidelines
  • Obtains Continuing Education Units (CEU’s) required for maintenance of State licensure to practice. 
  • Completes renewal process for State licensure timely to avoid expiration and submits copies of renewed license to Rehab Director and HR.  

Requirements

 

Minimum Qualifications:
Education: Graduate of an accredited Physical Therapy program and maintains current licensure as a Physical Therapist in Virginia or is a recent graduate and actively working toward licensure.

Experience:
Successful completion of required clinical affiliations, preferably including at least one rotation in the SNF setting.
