LH - Memory Care Manager (LPN)

As our Memory Care Manager you will be responsible for managing operations of the community’s Memory Care Unit by facilitating optimal quality of life for memory-impaired residents, including clinical and recreational care, in accordance with established policies, guidelines, and regulations.

Management Responsibilities

  • Supervises performance and behavior of staff holding them accountable for care-giving practices, job performance, customer service, record-keeping, dress code standards, attendance, policy compliance, etc. 
  • Provides “on the spot” training, guidance, coaching and counseling as needed to correct and improve staff performance including regular performance appraisals.
  • As necessary to correct improper job performance and/or behavior of staff, implements formal corrective action in compliance with established policy.   Included may be suspending a staff member and gathering / recording information regarding circumstances that lead to suspension.   
  • Participates in the interview process and selects or makes recommendations for selection of staff for department or unit of responsibility.
  • Collaborates with immediate supervisor to develop annual operating and capital budgets. 
  • Reviews employee time records to ensure compliance with labor budget and to address staff non-compliance with time-keeping requirements (like “punching in and out” appropriately)
  • Uses sound professional judgment to appropriately advise immediate supervisor or Executive Director of non-routine occurrences or issues.   

Other duties and responsibilities

  • Designs and implements programming specifically designed to meet the needs of memory-impaired resident population. 
  • Ensures that resident assessments are performed as needed to evaluate current and changing functional capacity of Memory Care residents.
  • Identifies changes in clinical condition and intervenes to ensure that services are appropriately adjusted to meet new needs.
  • Ensures that resident care needs are met with regards to activities of daily living, ambulation, hygiene and grooming, and medication administration.
  • In collaboration with the Activities Director, ensures that residents are provided person-centered therapeutic recreational activities designed to meet unique needs. 
  • Works collaboratively with immediate supervisor and others on the Memory Care team to develop and implement appropriate ways to address unusual / challenging resident behavior. 
  • Regularly communicates with family of residents to keep them informed of resident condition and needs and to involve them in developing and implementing unique approaches to care-giving.   
  • Ensures regulatory compliance on the Memory Care unit as well as compliance with established clinical systems of the community.   
  • Completes required clinical documentation in accordance with State regulations and facility policy.
  • Prepares and communicates work schedules for direct care staff in Memory Care.  
  • Maintains certification as a “Positive Approach to Care” coach.
  • Acts as coach for Memory Care staff performance of “Positive Approach to Care” techniques.   
  • Identifies and addresses gaps in knowledge / skills related to “Positive Approach to Care” for Memory Care staff. 
  • As requested by immediate supervisor or Executive Director, participates in educational and/or marketing events designed to promote the Memory Care unit.    

Requirements

 

QUALIFICATIONS

  • Must be a Licensed Practical Nurse (LPN) in the state of Virginia or compact state.
  • At least 2 years of experience as a LPN providing clinical care
  • At least 1 year of experience working with memory-impaired population in adult residential facility, adult day care center, nursing home, or licensed home care or hospice organization.
  • Current CPR certification
