LH - Wellness Nurse (LPN)

Job Description

 

As our Wellness Nurse you will be responsible for providing oversight to resident care on their shift.  This includes administering or overseeing the administration of prescribed medications in accordance with physician orders, providing guidance and supervision to Nursing Assistants and RMAs, responding to special needs and emergencies, and communicating appropriate information to residents, families, and others in the community.

Principle Duties and Responsibilities

  • Provides guidance and supervision to Registered Medication Aides (RMAs) ensuring that prescribed medication is administered within appropriate time frame and is documented as required by regulation and facility policy.  
  • Administers prescribed medication as necessary (when qualified RMA not available)
  • Provides guidance and supervision to Nursing Assistants who provide direct care services to residents.
  • Completes routine resident assessments and documentation as required by regulation/facility policy
  • Receives, transcribes, and implements or ensures safe and timely implementation of physician orders for residents on assigned unit. 
  • Performs resident assessments as needed to evaluate current and changing functional capacity of Memory Care residents.  
  • Reacts immediately to clinical emergencies involving residents by quickly assessing needs, providing nursing intervention as appropriate, arranging alternate services that may be required 
  • In accordance with physician orders, administers routine injections (insulin, etc.), obtains blood samples via venipuncture, and starts intravenous (IV) therapy.
  • Welcomes residents and families newly admitted to unit in a friendly and caring manner and provides information and education to facilitate smooth transition.
  • Functions as the facility-wide supervisor (designated staff person in charge) in absence of immediate supervisor and Executive Director.

Requirements

 

QUALIFICATIONS

  • Must be a Licensed Practical Nurse (LPN) in the state of Virginia or compact state
  • At least 1 year of experience preferred in a clinical role as a health care professional in an adult residential facility, adult day care center, acute care facility, nursing home, or licensed home care or hospice organization.
