The Staff Physical Therapist is responsible for duties including patient evaluation, program development, program implementation and utilizing techniques and modalities necessary to provide physical therapy services to individuals with acquired brain injury and other neurologic diagnosis at all levels of function. This physical therapist position is full time, primarily Monday through Friday. Work is performed at the CNS clinic(s) with some work at the residential treatment setting(s) and in the community.

A high degree of accuracy in documentation and observation, adaptability to a variety of situations, strong organizational and communication skills, decision making and an ability to evaluate situations as they occur is needed for this position.

Minimum Requirements


This position requires a high degree of accuracy in documentation and observation, strong organizational and communication skills, an ability to be flexible and adaptable to changing needs of staff and patients, and a strong background in the nature, treatment and identification of cognitive, language, auditory, or verbal disorders.

Initiates and maintains therapeutic programs. Administers initial evaluations, identifies primary impairments, administers appropriate outcome measures, develops and implements appropriate treatment plans. Requires visual and auditory acuity to evaluate, detect, and identify patient needs.

Implements patient treatment programs, including therapy, vestibular rehabilitation, home evaluations with recommendations, orthotic and assistive device recommendations, and therapeutic home programs. Executing these programs may include involving the patients in various types of mobility exercises, neurodevelopment sequence exercises, flexibility exercises and extremity manipulations. In addition, assists with the physical integration of individuals with brain injuries to various types of surfaces including hills, stairs, even and uneven surfaces. This very often requires an ability to provide patients with virtually all levels of assistance from standby to dependent assistance.

Provides pain relief modalities including application of hot packs, cold packs, "Omni stim" units, neuromuscular stimulation unit, transcutaneous nerve stimulation units. Other modalities could include the ability to perform light and deep massage.

Keeps constant check on progress to ascertain programs' effectiveness. Reviews and updates patient programs through interim evaluations. Requires visual, verbal, auditory acuity to evaluate and treat patients. Lifting, handling, carrying and manipulating patients weighing up to 200 pounds is required.

Instructs clinic aides of treatment plan to be completed daily. Supervises clinic aides and students and is available at all times to provide advice or treatment.

Provides training for residential and clinic staff on transfers, and provides supervision and training for mobility.

Ensures hydroculator is cleaned once a month or as necessary.

Performs reporting procedures.

  1. Submits monthly (or more frequent) conference reports, initial evaluations, discharge summaries and therapeutic home programs for assigned patients.
  2. Performs daily charting for patients treated and supervised.
  3. Reports verbally or in writing (according to patient/carrier/company needs) in conferences, staffings and other meetings to inform necessary personnel, insurance carriers, patients' families, patients, (as appropriate) or others of patient testing, treatment and progress. Requires visual ability, fine motor coordination, and knowledge of grammar and spelling to dictate and review/edit reports.
  4. Keeps informed of all patient programs in the department. Requires ability to read and understand written programs.

Consults with patients, families and therapists from various clinic disciplines to instruct, advise or inform them of procedures and tests. Plans clinical programs under the direction of a Clinical Case Manager assigned to a specific patient's case. Determines recommendations for length of treatment and intensity of the program.

Provides for the safety of patients, visitors, staff and self when interacting with patients who may have behavioral outbursts.

  1. Must be able to recognize patient escalation, via verbal, physical and visual cues.
  2. Must be able to attempt to verbally and physically de-escalate patient using Behavior Modification techniques according to each patient's program, which may include Professional Crisis Assault Training (Pro-ACT) such as evasive self-defense (ability to get out of the way) and controlling self-defense (restraints).
  3. Physical intervention and restraints can include calling for help, lifting, standing, walking, running, stooping, bending, kneeling, squatting, carrying, pulling, pushing and assisting with the restraint of patients weighing up to 150 pounds or more.

Performs toilet and chair transfers with assistance for patients up to 150 pounds utilizing approved transfer techniques.  Transfer techniques include standing, walking, stooping, bending, kneeling, squatting, lifting, carrying, pushing and pulling.  Patients requiring moderate to maximum assistance and weighing over 150 pounds are lifted and/or transferred by two or more persons.

Uses special equipment as required by each patient including, but not limited to wheelchairs, tracheostomy tubes, gastric feeding tubes, and external catheters, at any given time according to the outline of each patient's program.  May require find motor manipulation, lifting, reaching, carrying, pushing and pulling when working with equipment.

Maintains professional and ethical relationships with staff, patients, patient families, and other visitors.

Attends staff meetings, in-services, and all scheduled shifts.

Master's Degree or Doctorate level of education required.

Current licensure required or license-eligible in state employed.

Membership in APTA is desirable
