Description

 

Position provides assessment and referral services to adults who reside in a Nursing Facility, and an intellectual or developmental disability or dually diagnosed facility. Responsibilities include, but are not limited to performing assessments to determine whether identified individuals need nursing facility services, to identify alternate placement options when applicable, and to identify specialized services that may benefit the individual.  Incumbent must be able to identify service needs and a variety of community resources to address those needs; to negotiate and advocate with the service units for individuals; and develop effective working relationships with the individuals, family members and/or legally authorized representatives, Nursing Facility staff, LAR’s and other service providers; and to document services provided.  Must be highly organized and able to meet strict timelines for enrolling individuals into state Medicaid waiver programs, and for completing the PASRR process.  Must have effective verbal and written communication skills, problem solving abilities and an aptitude for teamwork.  Must be available to work weekends as needed. Extensive travel within the local service area is required.

Salary: $16.13/hourly

Minimum Requirements

 

Minimum Qualifications: Requires a Bachelor’s degree from an accredited college or university with a major in counseling, social work, or psychology, or criminal justice.with at least 1 year’ experience working with Individuals with Developmental Disabilities.  Must have a valid Texas driver’s license and acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas. 

Preferred Qualifications: Two years’ experience working in PASRR or with IDD/MI population in a Nursing Facility setting; knowledge of psychotropic medications and DSM V; proficient computer skills, including MS Word, Excel and Outlook; and bilingual skills (English/Spanish).
