This is a part-time temporary position with the potential to turn into full-time in the future.

As a Case Manager you will provide case management services to individuals and youth in transitional living. You will manage the client intake/discharge process, work with program participants to develop case plans/goals and monitor, guide and assist them to carry out case plan tasks. You will meet with program participants on a regular and communicate often with family members, professionals and outside agencies involved in client transition regarding client progress. You will complete assessments, determine eligibility for government and community services, make referrals and develop a comprehensive follow up system to maintain a contiguous path of services. In this role you will facilitate weekly client groups and/or client recreation activities, build new relationships with community organizations and employers, build and maintain collaborative relationships with a range of providers for current and future service and promote/represent the program at community gatherings. You will maintain accurate and current case files, program records and reports and other documentation.

Minimum Requirements

  • Four or more years of a combination of education and experience in working with similar clientele.
  • A Bachelor’s degree in social work or a related field is desired.
  • A valid driver's license, acceptable driving record and a Motor Vehicle Check.
  • Computer literate.
  • Strong written and oral communication and problem solving skills.
  • Strong organizational skills.
  • Able to demonstrate professional boundaries.
  • Ability to promote a positive, supportive and collaborative atmosphere.
  • Knowledgeable regarding community programs and referrals.
